Refer Someone to CRC
CRC accepts referrals from parole and probation departments, courts, the Ohio DRC Bureau of Community Sanctions, and directly from individuals seeking services. Our admissions team will work with you to determine eligibility and fit.
Once an individual is accepted, participation becomes mandatory if directed by court or Adult Parole Authority. Our goal is to make the referral process straightforward and supportive for everyone involved.

How Referrals Work
Submit Form
Complete the referral form below with the individual’s information and your contact details.
Review
CRC admissions staff reviews the referral and conducts an ORAS risk assessment to determine eligibility.
Placement
If accepted, CRC coordinates placement with the referring officer, court, or agency and establishes a start date.
Orientation
The resident arrives and begins orientation, is assigned a case manager, and a personalized program plan is developed.
Who We Serve
CRC provides rehabilitative residential services to adults under Adult Parole Authority supervision, Department of Rehabilitation and Corrections supervision, and Bureau of Community Sanctions. Both Transitional Control and Non-TC clients are accepted.
- Assessment via the Ohio Risk Assessment System (ORAS)
- Willingness to participate in all required programming
- Commitment to substance-free living (random testing required)
- Ability to seek or maintain employment during stay
- Compliance with all facility rules and supervision conditions
